Intuitive Surgical, Inc. (NASDAQ:ISRG) more or less invented modern robotic surgery with its da Vinci system, and for two decades it had the field to itself. The stock has now fallen about 35% from its high to roughly $395, yet it still trades near 46 times earnings and at $140 billion in value. So here is what you are buying at that price: a company the market expects to stay a near-monopoly, even as serious competitors finally arrive. The whole debate is whether that moat holds.

The Moat Math
The strength of Intuitive’s market position rests on procedural recurring revenue. About 86% of its revenue is recurring, from the instruments, accessories, and service tied to each procedure, and that stream grows as more operations happen. Its installed base reached 11,710 da Vinci systems by mid-2026, up 12% in a year, and worldwide procedures are growing around 14%. Once a hospital acquires a surgical console, it purchases specialized instruments for every surgery performed. In addition, surgeons spend years training on da Vinci hardware during their medical education, making the switching slow and costly for the hospital networks. This is the expectation baked into 46 times earnings: that double-digit procedure growth and this high-margin recurring base largely continue.
The Bull Case
Here is the case for the buyers. The switching costs are enormous while the recurring revenue makes earnings unusually steady for a device maker. The procedure runway is long, with new surgical uses and under-penetrated markets abroad still opening up. The newer da Vinci 5 is driving higher usage, with utilization running about 11% above the older system. The bulls also argue that the marketing efforts from well-capitalized competitors could increase total robotic surgery adoption across hospitals that currently rely on conventional open surgery, expanding the total market for all participants.
The Bear Case
The bears, however, think the moat is finally being tested. Intuitive is facing heavy competition for the first time from rivals including Medtronic’s Hugo and Johnson & Johnson’s Ottava. Both got clearance from US regulators. Specifically, Ottava’s space-saving design targets exactly the hospitals Intuitive relies on. New entrants into the market could pressure system placements and pricing. And at 46 times earnings, any slowdown in placements or pricing could prompt further valuation compression. In addition, multi-million-dollar capital placements remain sensitive to hospital budget constraints and borrowing costs. The 34% drop suggests the market is already repricing that competitive risk.

The Bottom Line
So, the real question is whether Hugo and Ottava dent Intuitive Surgical, Inc.’s growth and pricing, or mostly expand the robotic-surgery market, which could even help the leader? For a growth investor, the recurring-revenue engine and long procedure runway are the draw, if the moat holds. For a value investor, 46 times is cheap only if the company manages consistent market dominance and stable gross margins. An income investor finds little here, since Intuitive pays no dividend.
