ProKidney (PROK) Nears A Pivotal Kidney Therapy Verdict

On August 10, ProKidney Corp. (NASDAQ:PROK) reported second-quarter financial results that doubled as a status update on the biggest bet in its history: a cell therapy for chronic kidney disease that is now one step closer to a verdict. The clinical-stage company completed enrollment for the analysis that will decide whether its treatment gets an accelerated approval shot, a milestone years in the making for a drug aimed at a disease with few good options. With topline results still nearly a year away, the quarter is really about whether ProKidney can stay funded and focused long enough to get there.

A Trial Finally Nears Its Finish Line

ProKidney finished enrolling the roughly 320 patients who will make up its Phase 3 PROACT 1 accelerated approval efficacy analysis, clearing the way for a pivotal eGFR slope readout the company expects in the second quarter of 2027. That analysis sits inside a larger, randomized, sham-controlled study of about 470 patients with advanced CKD and type 2 diabetes, testing whether up to two injections of rilparencel can slow the decline in kidney function enough to matter. The company also said it remains on track to complete full enrollment for that larger group in the second half of 2026, with a confirmatory analysis on tap for the back half of 2029.

Alongside the trial progress, ProKidney added Kenneth Locke as Chief Technical Officer in June. Locke brings more than 25 years in R&D, manufacturing, and supply chain, including a stint as head of technical operations at Carisma Therapeutics and earlier roles at Celgene and Novartis building out cell therapy manufacturing. For a company with no approved product yet, bringing in someone who has already built the supply chain for first-in-human cell therapies is a hire aimed squarely at what comes after a positive trial result.

The Cash Clock Is Ticking Louder

Getting to that 2027 readout costs money, and the quarter showed the bill going up. Research and development spending climbed to $36.1 million from $25.9 million a year earlier, an increase the company tied mostly to clinical study and manufacturing costs tied to PROACT 1. General and administrative costs actually fell to $12.4 million from $14.0 million, helped by lower compensation costs as older equity awards vested and some severance costs faded. That was not enough to offset the R&D increase, and the net loss before noncontrolling interest widened to $46.4 million from $37.0 million a year earlier.

Cash, cash equivalents and marketable securities dropped to $181.6 million at the end of June from $224.9 million just three months earlier, a roughly $43 million drawdown in a single quarter. ProKidney says that balance supports operations into mid-2027, which lines up almost exactly with the expected PROACT 1 topline data. That leaves little room for delay: any slip in the trial timeline would collide directly with the point at which the company’s funding runs out.

Where The Market Is Placing Its Bets

The number of hedge funds holding ProKidney fell sharply, from 21 in the prior quarter to just 7, a steep drop in institutional conviction heading into the pivotal data window. Short interest sits at 13.83% of the float, a level that signals a real and organized bear camp rather than routine hedging. Taken together, fewer funds are willing to hold the stock through the next year, while a meaningful slice of the market is positioned for it to fall further.

A Story That Turns On One Readout

ProKidney has done the unglamorous work of finishing enrollment and hiring the manufacturing expertise a future launch would require, and its cash position is calibrated to last exactly as long as it needs to. But that calibration leaves no margin, and departing hedge funds alongside double-digit short interest show the market is not fully convinced the timeline holds. For the bull case to play out, the eGFR slope data next year needs to arrive on schedule and support accelerated approval.
