Could Mounjaro’s New Heart Benefit Unlock the Next Growth Phase for Lilly?

Eli Lilly and Company’s (NYSE:LLY) Mounjaro has already become a major growth driver in diabetes treatment, and its latest FDA approval could strengthen that position by expanding what physicians can use the drug to accomplish.

The FDA approved Mounjaro to reduce the risk of heart attack or stroke in adults with type 2 diabetes who face a high risk of cardiovascular events. The decision broadens the drug’s role beyond controlling diabetes and gives Lilly another way to demonstrate its value in a patient population frequently affected by cardiovascular complications.

Bull Case

The strongest argument for the expanded approval is the supporting clinical evidence. In a large head-to-head study, Mounjaro outperformed Trulicity, which is Lilly’s older diabetes medicine, in reducing the risk of heart attack and stroke. The study found that Mounjaro reduced the risk of major adverse cardiovascular events by 8% more than Trulicity. A direct comparison with another active diabetes treatment provides a more demanding test than a study comparing Mounjaro solely with a placebo.

The FDA’s decision gives Lilly an additional benefit that physicians can consider when selecting a treatment for eligible adults with type 2 diabetes. Instead of being evaluated only for its effects on diabetes, Mounjaro can now be prescribed with an approved indication for reducing heart attack and stroke risk in high-risk patients. The approval also arrives while Mounjaro is experiencing substantial commercial momentum, with sales increasing 91% year over year to $9.94 billion during the second quarter, supported by higher volumes across global markets. The new indication could reinforce that growth by expanding the clinical reasons for prescribing the drug within its eligible population.

Lilly is also extending the same underlying medicine across different metabolic conditions. Mounjaro is marketed for diabetes, while the obesity version is sold as Zepbound in the United States. This allows the company to address diabetes and weight management through separate brands while continuing to investigate additional potential uses. More broadly, Lilly is testing its GLP-1 medicines in conditions extending from cardiovascular disease to liver disorders. The Mounjaro decision provides tangible evidence that the strategy can produce regulatory approvals beyond the drugs’ original metabolic benefits.

Bear Case

It is significant to note that the approval applies to a specific group rather than every Mounjaro patient, and covers adults with type 2 diabetes who are considered at high risk of cardiovascular events. It should not be presented as evidence that Mounjaro has an approved cardiovascular-risk indication for people without diabetes or for lower-risk patients.

Commercial expectations are already substantial following Mounjaro’s 91% sales increase. The new indication strengthens the drug’s clinical profile, but Lilly has not disclosed how much additional revenue it expects from the approval. Some eligible patients may already be receiving Mounjaro for diabetes, limiting the number of entirely new users created by the expanded label.

Lilly also operates in a market where cardiovascular benefits are becoming increasingly important, making competition a significant factor to consider. Novo Nordisk secured FDA approval in 2024 for Wegovy to reduce cardiovascular risk in overweight or obese adults without diabetes. Its diabetes pill Rybelsus received a cardiovascular-risk indication for diabetic patients in 2025.

Conclusion

Mounjaro’s expanded approval adds an important cardiovascular benefit to a drug already generating strong sales growth. The head-to-head result against Trulicity gives Lilly a compelling clinical argument when physicians evaluate treatments for high-risk adults with type 2 diabetes.

The commercial effect is less certain because Mounjaro is already widely used and the indication targets a defined population. Even so, the decision strengthens the drug’s position and advances Lilly’s strategy of extending its metabolic medicines into cardiovascular care.
