Sionna Therapeutics, Inc. (NASDAQ:SION) announced on September 14 that its board approved an approximately 46% workforce reduction as it concentrates development on SION-451 plus SION-2222 for cystic fibrosis.
Sionna Therapeutics, Inc. ended the second quarter with approximately $268.3 million in cash, cash equivalents, and marketable securities. Management estimates that the workforce reduction, discontinued investment in SION-719 as an add-on to standard treatment, and other savings will extend cash runway into the second half of 2029. Restructuring and related charges are expected to total approximately $6.4 million.
Patient evidence must now justify the remaining investment.

Sionna Therapeutics, Inc. plans to begin the AscenSION CF Phase 2a study in the first quarter of 2027. Adults with two copies of the F508del mutation would switch from Trikafta to SION-451 plus SION-2222 for 28 days. The open-label trial will assess sweat chloride, safety, and pharmacokinetics, including drug exposure in the body.
Testing the combination as a replacement directly addresses its intended use. The earlier study evaluated a different candidate, SION-719, added to Trikafta. Sionna Therapeutics, Inc. can now test its preferred combination without the same background regimen.
Healthy-volunteer results provide a basis for proceeding. The Phase 1 combination study achieved its safety, tolerability, and drug-exposure objectives. SION-451 twice daily with SION-2222 once daily was generally well tolerated, although one participant discontinued because of rash.
Concentrating resources creates a clearer capital-allocation test. Healthy-volunteer findings establish neither efficacy in cystic fibrosis nor a favorable long-term safety profile. A 28-day study can reveal an early biological signal, but durable patient benefit requires further evidence.
The earlier failure remains significant. SION-719 produced a mean placebo-adjusted sweat chloride change of just -1.0 mmol/L, missing the trial’s key activity endpoint. Sweat chloride measures CFTR protein function, rather than directly establishing improvements in symptoms or lung function.
Sionna Therapeutics, Inc. subsequently reported a mean placebo-adjusted sweat chloride change of -8.6 mmol/L in a post hoc subset of 11 of 14 evaluable participants. The analysis excluded three participants with exposure patterns consistent with nonadherence and used baselines measured at the start of each treatment period. These exploratory findings cannot overturn the original result or establish that SION-451 plus SION-2222 will work.
The open-label replacement study also offers less protection against bias than a blinded, randomized comparison. Adherence and sweat chloride variability remain important when interpreting results.
Financial execution adds another risk. A 46% workforce reduction does not translate automatically into an equivalent decline in spending. Trial, manufacturing, and external-service costs continue, while losing key employees could delay execution. The projected runway depends on achieving savings while preserving the capabilities needed to advance development.
