Pfizer Inc. (PFE) vs. Eli Lilly and Company (LLY): The Growth vs. Value Play in Pharma

Pfizer (NYSE:PFE) and Eli Lilly and Company (NYSE:LLY) offer unique divergent investment opportunities in the pharmaceutical industry. While one acts as a value and income play, the other stands out as a solid growth investment supported by rapid expansion in obesity and diabetes franchises.

Pfizer Value Investment Play

Pfizer (NYSE:PFE) has seen its valuation drop significantly, to the extent of becoming a value investment play pursued by income-focused investors. The stock has been under pressure owing to waning COVID-19 product sales and issues compounded by potential loss of exclusivity on key drugs.

Nevertheless, a robust drug portfolio, substantial cash-generation potential and a high dividend yield continue to strengthen its sentiment. Its valuation is also significantly below that of many large pharmaceutical peers as the stock trades at about 8.7x trailing adjusted earnings and with a low sales multiple of 2.2x.

A solid dividend payout record is another important part of the investment thesis. The company currently pays $0.43 per share quarterly, equivalent to $1.72 annually, resulting in a dividend yield of around 7%.

Investment  Risks Amid Value Play

The largest challenge is the continued normalization of Pfizer’s COVID franchise. The company expects revenue from its COVID-19 products to decline by approximately $1.5 billion in 2026. It also expects approximately another $1.5 billion of negative revenue impact in 2026 from products facing generic or biosimilar competition following the loss of patent or regulatory exclusivity.

The longer-term patent cycle is a big concern as key drugs led by Eliquis and Ibrance are at risk of losing their exclusivity protection. Consequently, the company is under pressure to replace mature-product revenue while investing in the pipeline.

Pfizer is also attempting to build a position in the obesity-drug market through its acquisition of Metsera and its broader GLP-1 pipeline. However, it is entering a market where Eli Lilly and Novo Nordisk A/S (NYSE:NVO) already have significant commercial scale and established brands.

Eli Lilly Growth Thesis

Eli Lilly and Company (NYSE:LLY), on its part, has delivered rapid revenue growth by gaining significant market share in diabetes and weight-loss drugs. That was evident as first-quarter revenues jumped 56%, supporting the growth thesis.  The company also hiked its full-year revenue guidance in anticipation of growth momentum for the year.

The company is also investing heavily to expand manufacturing capacity. That spending could weigh on near-term free cash flow, but it is intended to increase supply and support the company’s ability to meet growing demand for its obesity medicines.

Consequently, its investment case depends on continued growth in earnings and cash flow, with its dividend providing only a modest additional return.

Eli Lilly Investment Risks

The primary risk for Lilly is valuation, as it trades at roughly 40.7x trailing earnings, 14.3x sales, and 29.3x EV/EBITDA. While the metrics represent a substantial premium, they suggest high expectations for continued growth.

Blockbuster drugs including Mounjaro and Zepbound account for a significant chunk of the company’s total revenue. The high reliance could pose significant risks if the drugs lose exclusivity. Additionally, the company is under intense pricing pressure.

Competition is another consideration as companies invest heavily in obesity treatments, amid cost and accessibility pressures. Consequently, competition pressure could eventually weigh on Lilly’s margins and market share.

Hedge Fund Positioning and Short Interest

Lilly enjoys strong hedge fund holdings despite a modest decline in the recent quarter. 132 hedge funds holding stakes in the company in the first quarter, a slight drop from 137, according to the Insider Monkey database. Fisher Asset Management, Adage Capital Management and AQR Capital Management increased their positions by 6%, 15% and 69%, respectively.

Pfizer (NYSE:PFE) was held by 83 hedge fund portfolios at the end of the first quarter, up from 81 in the previous quarter. Fisher Asset Management, Marshall Wace and Unisphere Establishment increased their Pfizer positions by 103%, 28% and 266%, respectively.

Short interest also remains relatively modest for both companies. As of July 15, 2026, approximately 10.28 million Lilly shares were sold short, equivalent to 1.09% of the public float. Pfizer had short interest equivalent to 2.89% of its public float as of June 30.

The figures suggest that neither stock is facing exceptionally heavy short positioning, although short sellers have a comparatively larger position in Pfizer.

Bottom Line

Pfizer (NYSE:PFE) and Eli Lilly present sharply different investment cases offering different exposure in the pharmaceutical space.

Eli Lilly and Company (NYSE:LLY) offers a strong growth profile owing to a competitive edge on obesity and diabetes drugs. Expanding manufacturing capacity and a broader pipeline also provide multiple avenues for continued growth.

Pfizer, on the other hand, offers the more compelling value and income proposition. Its low valuation and roughly 7% dividend yield provide a significant contrast with Lilly. Still, the discount exists for reasons: declining COVID-related revenue, upcoming loss-of-exclusivity pressures and the need for its pipeline and cost-reduction efforts to translate into sustainable earnings growth.

While we acknowledge the risk and potential of PFE as an investment, our conviction lies in the belief that some AI stocks hold greater promise for delivering higher returns and doing so within a shorter time frame.
