Item 1.  Security and Issuer
This Statement relates to shares of common stock with $0.001 par value per share of Fortem Resources Inc. (the “Issuer”). The principal executive offices of the Issuer are located at Suite 820, 906 12th Avenue SW., Calgary Alberta T2R 1K7, Canada.
Item 2.  Identity and Background
(a)
Name:  Angela Mainardi (the “Reporting Person”).
(b)
Residence or business address:  1503 Commercial Drive, Vancouver, BC  V5L 3Y1.
(c)
The Reporting Person is a business person.
(d)
The Reporting Person has not, during the last five years, been convicted in a criminal proceeding (excluding traffic violations or similar misdemeanours).
(e)
The Reporting Person has not, during the last five years, been a party to a civil proceeding of a judicial or administrative body of competent jurisdiction and as a result of such proceeding was or is subject to a judgment, decree or final order enjoining future violations of, or prohibiting or mandating activities subject to, federal or state securities laws or finding any violation with respect to such laws.

The Reporting Person is a citizen of Canada.
Item 3.  Source and Amount of Funds or Other Considerations
On October 20, 2018, Pacific Petroleum, LLC sold 500,000 shares of common stock of the Issuer to a purchaser at US$0.0038 (CDN$0.005) per share for aggregate proceeds of US$1,907.75 (CDN$2,500.00).
On October 20, 2018, Pacific Petroleum, LLC sold 2,000,000 shares of common stock of the Issuer to a different purchaser at US$0.0038 (CDN$0.005) per share for aggregate proceeds of US$7,631.00 (CDN$10,000.00).
The Reporting Person is the beneficial owner of all of the issued and outstanding membership interests of Pacific Petroleum, LLC.
Item 4.  Purpose of Transaction
The response to Item 3 is responsive to this Item.
As of the date hereof, except as described above, the Reporting Person does not have any plans or proposals which relate to or would result in:

 
the acquisition by any person of additional securities of the Issuer, or the disposition of securities of the Issuer;
     
 
an extraordinary corporate transaction, such as a merger, reorganization or liquidation, involving the Issuer or any of its subsidiaries;
     
 
a sale or transfer of a material amount of assets of the Issuer or any of its subsidiaries;
     
 
any change in the present board of directors or management of the Issuer, including any plans or proposals to change the number or term of directors or to fill any existing vacancies on the board;
     
 
any material change in the present capitalization or dividend policy of the Issuer;
     
 
any other material change in the Issuer's business or corporate structure;
     
 
changes in the Issuer's charter, bylaws or instruments corresponding thereto or other actions which may impede the acquisition of control of the Issuer by any person;
     
 
causing a class of securities of the Issuer to be delisted from a national securities exchange or to cease to be authorized to be quoted in an inter-dealer quotation system of a registered national securities association;
     
 
a class of equity securities of the Issuer becoming eligible for termination of registration pursuant to Section 12(g)(4) of the Act; or
     
 
any action similar to any of those enumerated above.
Item 5.  Interest in Securities of the Issuer

The aggregate number and percentage of common stock of the Issuer beneficially owned by the Reporting Person is 17,500,000 shares of which shares are held indirectly by Pacific Petroleum, LLC, or approximately 14.37% of outstanding common stock of the Issuer, based on 121,771,156  shares issued and outstanding as of November 1, 2018.
Angela Mainardi exercises investment power over the shares of common stock held by Pacific Petroleum, LLC.

The Reporting Person has the sole power to vote or direct the vote, and to dispose or direct the disposition of 17,500,000 shares of common stock of the Issuer.  See also Items 3 and 5(a).
